Talent Show

I will be performing the following act ________________________________

I will need the following equipment (mats, mics, CD player, etc.)

My signature is my promise that I will continue to practice and polish my act. I also acknowledge that schoolwork comes first. If I am not able to maintain good grades and good behavior, I will have to withdraw from the talent show. I also agree to attend every assigned practice. If I am not able to attend rehearsal and have a very good reason, I must speak with Mr. Corey or Ms. Garino in advance in order to be excused. More than one unexcused absence is grounds for dismissal. A list of practices can be found on the other side of this sheet.

I also promise to attend the entire dress rehearsal on ........TBA, though it could end earlier. I understand that I will not be able to participate in the show if I cannot make this commitment.

The talent show will be on .....TBA. An exact time will be announced soon.
