I would like to remind the parents of kindergarten and fifth grade students that I need a copy of a recent physical for your child's medical records. The fifth grade physical form must have documentation of scoliosis screening. The kindergarten form must be sent in within the first six months of school.
Please remember that NO MEDICATION can be given to your child in school without a doctor's order. Even if it is an over the counter medication, I still need an order from your child's physician. I would also like to remind parents to PLEASE keep your child home from school for AT LEAST 24 hours after they have been sent home with a fever or vomiting. Even if they don't have a fever or vomiting the next morning, they should be fever and vomit free for 24 hours before returning to school.
